Policy for Teens Learners Permit

Forms Required for Permit Test at DMV

1. Proof of age: birth certificate, U.S. passport or alien registration card.
 
2. Social Security Card
 
3. Proof of Address (something received by mail with a postmark within 90 days). Example: magazine, postcard, or bill.
 
4. Parental consent form (supplied by DMV)
 
5. Fee: $40.00 Cash, credit cards, check or money order (make check payable to DMV).

6. After successfully passing vision screening and 20 out of 25 questions, you will be charged $18.00  for your photo permit.
 
Rules

You must be at least 16 years old, read the Connecticut drivers manual (link for manual to the right of page), pass an eye exam and a 25-question test.
